-Neck midline
-Mechanical hyperventilation
-Mannitol
-3% Saline
-EVD
-Barbiturates
-Surgery
ICP Normal Value
0-15 mm Hg
CPP calculation
MAP - ICP = CPP
CPP normal range
60-100 mm Hg
*70 or higher in those with abnormal brain pathology
Autoregulation
Autoregulation is the ability of cerebral blood
vessels to adjust their diameter to arterial
pressure changes within the brain.
Glascow-Coma Scale
-Minimum Score 3
-At 8 intubate
Transient Ischemic Attack (TIA)
Transient episode of neurologic dysfunction – no acute infarction of brain
Ischemic Stroke
inadequate blood flow to brain
• Thrombotic
• Embolic
*Most common type of stroke
Hemorrhagic Stroke
bleeding into brain tissue
• Intracerebral
• Subarachnoid
*Worst headache they’ve ever had!
KNOW SLIDE 18!!!
Right-brain damage
Stroke on right side of brain
-Paralyzed left side -hemiplegia
-Left-side neglect
-
FAST
Face -droop, smile
Arms- close eyes and lift arms
Speech
Time -exact onset of symptoms
Know 12 cranial nerves
Ischemic Treatments
Stent
• TPA – strict time rules & contraindications
• Long term anticoagulants
Hemorrhagic Treatment
• Resection
• Aneurysm clip
• Evacuation of hematoma
Cheyne-Stokes
rhythmic up and down of rate & depth of breaths
-Rapid shallow breathing and than a period of apnea
ICP Diagnostic
> 20 mm Hg for 5 minutes or longer
EVD:
external ventricular drain
-monitor ICP
-drain of CSF
EVD Fluid Used
Normal Saline WITHOUT preservatives
EVD Monitoring
-NO meds NO drawing blood
-Drain off as needed
*when ICP is increased >20 for 5 mins
ICP Positioning
ICP Suctioning
ICP Neuro Assessment
