Icd10 Flashcards
The icd includes a look at the general
Health of the population and is used to monitor incidence and prevalence. It is the Icd that provides mortality number of deaths and morbidity number of diseases for the world health organisation. Addresses all diseases not just those involved in mental disorders
The icd is produced by the
WHO world health classification an agency oc the unuted nations where’s the dsm is produced by the apa American psychiatric society wguch is the single nation professional body
The id us a
Free and open recourse whereas the dsm is revenue resource for the apa
The icd is available to
Countries and service providers whereas the dsm is available to psychiatrists only
The icd is
Multilingual and multidisciplinary where’s the dsm is us dominated and in the English lang only
The icd covers all
Health disorders while the dsm covers only mental disorders
Reliability of icd10 with regard to schizophrenia
Jakobsen et al 2005
Used a random sample of Danish in - patients diagnosed with schizophrenia and s sample out if out patients with a history of psychosis to look at the reliability of the icd10 with regard to schizophrenia an icd10 diagnosis showed 93 sensitivity and 87 percent predictive vKjr when it came to diagnosis of schizophrenia and showed good agreement between the icd10 and diagnosis using another measure
Hiller et al 1992
Compared the icd10 with the dsm 111r to look at validity and reliability. The researchers used both ckassification system on the same set of patients who were suffering from affective and psychotic disorder. The icd10 gave higher reliability for all disorders looked at expect doe bipolar disorder
Cheniaux et al 2009
Looked at various diagnoses including schizophrenia bipolar depression and unipolar depression. They used inter rater reliability and their aim was to look at reliability of diagnosis using the dsm iv and icd10. Two trained psychiatrists h
Using interviewing to assess the diagnosis of 100 patients. They found that schizophrenia was more frequent using the icd10 than if using the dsm iv.
Strength of reliability of icd10 on diagnosis reliability
. Studies tend to show that the icd10 is a reliable measure of schizophrenia and it also compares well in terms of reliability with the dsm 111r hiller et al al 1992 Jakobsen 2005
.sruduss use inter rater reliability which means more than one person uses the ckassiciation system or system with the same patient and when they come up with the same diagnosis reliability can be claimed there are careful controls to ensure that ensure that eaters work independently to avoid bias
Weakness of reliability of icd10
Schizophrenia was diagnosed more frequently when icd10 was used then when dsm iv was used which suggests a lsxk of reliability between 2 systems
. Similarly schizo affective disorders wasn’t reliability diagnosed according to cheniaux et al so reliability is in doubt for some disorders related to schizophrenia
. The reliability figures were around 0.50 agreement which leaves a lack of agreement also to be accounted for
Validity of icd10 diagnosis schizophrenia
Pihlajamma et al 2008
Looked at the validity of diagnosis using the dsm iiir the dsm iv and icd10. The study looked at 877 people born in helinski in Finland between 1951 and 1960 who had at least one diagnosis of schizophrenia. Using the dsm iv and the register 70 to 80 percent. Concluded that register diagnosis and comparing them using icd10 and the dsm it could be seen that diagnosed aren’t valid
Janson et al 2002
Wanted to look at the icd10 criteria for schizophrenia compared with other diagnostic systema. 155 patients. The icd10 seems to be less valid than the icd9
Strengths of the validity of the icd10
In general when the icd10 is used to diagnose schizophrenia the diagnose matches a diagnosis using a different system which suggest that the icd10 is valid
. According to janson et al 2002 the icd10 and the dsm in cabs an excess agreement in diagnosis which is high thought if may be said that the study reported differences
Weaknesses validity if icd10
. Janson et al 2002 found that different classification system focused on different symptons and feature in schizophrenia which threatens the validity of the 2 systems. There is a general suggestion that there is a different focus and so validity of diagnosis is less likely
. Studuss of validity of diagnosis of schizophrenia are hard to carry out given differences in disorders such as whether the core schizophrenia spectrum disorder is considered. In general it seems that the complexity of the disorders is an issue when it comes to claiming validity of diagnosis. Ellasob and Ross 1995 suggest that people diagnosed with dissociative identity disorder have more features and symptons of schizophrenia than those with schizophrenia. These were positive symptons
