When coding Glaucoma assign the appropriate
Eye & Stage
When the Type & Stage are the Same code 1 for both
Bilateral Glaucoma
Assign as many codes from Category H40 for Glaucoma to identify the Type of Glaucoma, the affected eye, & the stage
Assigning Glaucoma codes
Use 2 codes for each Eye, & assign the appropriate stage at the 7th character
Bilateral Glaucoma w/ DIFFERENT Type, & Stage
Pt. admitted w/ Glaucoma & Stage evolves during the admission
Assign the HIGHEST Stage
Clinical documentation must indicate the Stage is unable to be determined, then assign the 7th character as “4”.
Indeterminate Stage Glaucoma
Blindness
H54.3
If documented as Blindness or Low Vision in one eye, & the visual impairment category IS NOT documented use
H.54.6- Unqualified Visual Loss, One Eye
A condition of increased pressure within the eyeball, causing gradual loss of sight
Glaucoma
A disease of the eye in which the pressure of fluid inside the eyeball is abnormally high, caused by an obstructed outflow of the fluid.
H40.11x1- Open-Angle Glaucoma
Increased pressure can lead to damage the optic nerve, and lead to partial or complete blindness
Open Angle Glaucoma- H40.11x1
Is least common, but can cause a sudden buildup of pressure in the eye.
Angle-Closure Glaucoma
The angle between the iris & corneas may be too narrow
Chronic or Acute Glaucoma
