Current Procedural Terminology: set of codes, descriptions and guidelines intended to describe procedures & services preformed by physicians & other HCP or entities.

(99201-99499) appears at the beginning of the listed procedures and are used by most physicians in reporting significant portion of their services
Specific guidelines
are presented at the beginning of each section
Add-on codes
additional or supplemental procedures are designated as add-on codes with “+” symbol preformed by the SAME physician.
modifiers
provides the means to report that a service or procedure has been performed has been altered by some circumstance but not changed in its definition or code.
Place of service
some codes have a specified place of service and instructions specific to that place of service.
Special report
a service that is rarely provided, unusual, variable or new many require a special report
Time
is the face-to-face time with the pt
A unit of time
is attained when the mid-point has passed. ie: an hour is attained when 31 minutes have elapsed.
When another service is performed concurrently with a time-based service
the time associated with the concurrent service should not be included in the time used for reporting time-based service.
Disruption of a service
DOES create a new initial service.
