What are the symptoms of IBD?
increased bowel frequency rectal bleeding weight loss dehydration abdo pain
What are the investigations for IBD?
DRE + GI exams colo/sigmoidoscopy + biopsy Faecal calprotectin AXR CT scan Stool culture
What is the treatment for IBD?
fluid resus
pharmacological- crohn’s + UC
surgery
abx- diverticulitis
What are the complications of IBD?
bowel perforation toxic megacolon dehydration leading to renal failure severe weight loss/malnutrition systemic sepsis
What is colonic diverticulum?
protrusion of mucosa and submucosa through muscular layers of the bowel wall
Where in the colon are diverticula usually located?
left colon/sigmoid colon
Is rectal bleeding more common in ulcerative of CD?
in UC
What is the treatment of diverticulitis?
usually resolved with antibiotic treatment but may require surgery- sigmoid colectomy +/- stoma
What is the treatment of ischaemic colitis?
usually resolves with supportive treatment but can require colectomy/hemicolectomy
What is the treatment of UC/Crohn’s?
medical- steroids and immunosuppresion + surgery?
Name two indications for surgical intervention in IBD?
toxic megalon and medically resistant disease
What is the difference between diverticulitis, diverticulosis, and diverticular disease?
Diverticulosis= presence of diverticula
Diverticulitis= inflammation and infection of diverticula
Diverticular disease= presence of diverticula + abdominal pain
What is a positive qFIT associated with?
colorectal cancer, however can be positive in diverticular disease
Why are you worried about urinary symptoms in patient with diverticular disease?
fistula can develop and penetrate bladder in men and vagina in women, therefore recurrent UTIs that don’t improve with abx can be a sign of diverticular disease
