Name the causes of fasting hypoglycaemia.

- Exogenous drugs - insulin, oral hypoglycaemics, ACE-i, beta blockers
- Pituitary diseases
- Liver diseases (inability to breakdown glucagon)
- Addisons disease
- Insulinoma
- Non-pancreatic neoplasms

Name the causes of post prandial hypoglycaemia
A
- Bariatric surgery
- T2DM
- -> Rapid increase in blood glucose –> rapid increase in insulin –> rapid decrease in blood glucose

Name the common presentations of hypoglycaemia.
A
- Tremors
- Palpitations/tachycardia
- Confusion
- Behavioural change e.g. aggressiveness
- Sweating
- Seizures

What suggest an insulinoma?
A
Whipple’s triad
- Symptoms of hypoglycaemia
- Low blood glucose level
- Relieve of symptoms after glucose is raised

What blood glucose level must patient be treated?
A
<4mmol/L

What is the management plan for hypoglycaemia (if blood glucose <4mmol/L)?
A
- 15g of fast acting carbohydrate
- Measure blood glucose to make sure its above 4mmol/L
- Give long acting carbohydrate e.g. high fibre starch to prevent recurrence
