What is the function of prolactin
- Stimulates lactation and breast development
- Inhibit GnRH synthesis - inhibits ovulation in females + spermatogenesis in males (decreases FSH/LH as a response)
- Increase dopamine secretion
Explain the relationship between prolactin and dopamine
dopamine inhibits prolactin
What secretes prolactin?
anterior pituitary
what do raised prolactin levels lead to?
hypogonadism
infertility
osteoporosis (hence ↓LH/FSH and ↓testosterone or oestrogen)
What level is a raised prolactin?
> 390mu/L
What are causes of hyperprolactinaemia?
- XS production from pituitary e.g. prolactinoma
- Disinhibition by compression of the pituitary stalk reducing local dopamine levels
- dopamine antagonists
Give physiological causes of raised prolactin
pregnancy
breastfeeding
stress
What drugs cause raised prolactin?
metoclopramide haloperidol methyldopa oestrogens ecstasy antipsychotics
What diseases cause raised prolactin?
Prolactinoma
Stalk damage - pituitary adenomas, surgery, trauma
Hypothalamic disease - craniopharyngioma
Other: hypothyroidism, chronic renal failure
What are the sx of hyperprolactinaemia in females?
- amenorrhoea/oligomenorrhoea
- infertility
- galactorrhoea
- ↓libido, ↑weight, dry vagina.
What are the sx of hyperprolactinaemia in men/
ED
↓facial hair
Galactorrhoea
May present late with osteoporosis or local pressure effects from the tumour
What are the tests for hyperprolactinaemia?
Basal PRL LFTs U&Es Pregnancy test MRI pituitary
What is the treatment of prolactinoma?
- Dopamine agonists - cabergoline, bromocriptine
2. Surgery - trans-sphenoidal (if cannot tolerate or fail to respond to medical therapy)
