Pituitary tumour (95%) Hyperplasia

What is growth hormone also known as?
A
Somatotropin

Explain the GH axis
A
Hypothalamus releases GHRH which stimulates anterior pituitary to release GH
GH stimulates liver to release IGF (insulin like growth factor 1) which stimulates bone + tissue growth

What are the sx
A
- Facial features - frontal bossing, big nose, jaw enlargement, prognathism, separation of teeth, macroglossia
- Big hands + feet
- XS sweating + oily skin
- Features of pituitary tumour - hypopituitarism, headaches, bitemporal hemianopia
- Carpal tunnel syndrome
- Proximal weakness + arthropathy
- OSA, galactorrhoea

What are the complications?
A
HTN
DM
Cardiomyopathy
Colorectal cancer

Q
What tests would you do?
A
- Serum insulin-like growth factor 1 (IGF-1) - elevated
- Oral glucose tolerance test - lack of suppression of GH
- Random serum GH - not v reliable as varies during the day
- MRI pituitary fossa, look for hypopituitarism, visual fields + acuity, ECG, echo, old photos

What is the treatment of acromegaly?
A
- Trans-sphenoidal surgery
- if fails - somatostatin analogues (octeotride) and/or radiotherapy
- GH antagonist - pegvisomant
