Early morning/AM care
Risk factors for skin impairment
Immobilization, reduced sensation, nutrition and hydration alterations, secretions and excretions on the skin, vascular insufficiency, external devices, altered cognition.
After breakfast assist by offering a bedpan or urinal to patients confined to bed, provide bath or shower, pericare, oral care, change patients gown, change bed linens, straighten up bedside
Routine morning care
Includes washing he hands, face, oral care, bedpan or urinal, straightening linens
Afternoon care
Changing soiled bed linens, gowns, wash face and hands, oral care, bed pan and offer beverage
Evening or hour before bedtime care
Bath administered to totally dependent patient in bed
Complete bed bath
Bed bath consisting of bathing only parts that would cause discomfort if left unbathed
Partial bed bath
Involves bathing from a bath basin or sink with patient sitting in a chair
Sponge bath
Contains several cotton cloths hat are pre moistened in a solution of no rinse surfactant cleanser and emollient
Bag bath/travel bath
