HY Headache types Flashcards
Trigeminal neuralgia ?
- Knife like pain
- Lating <1minute
- Part of brain affected?=PONS
- PONS=CN 5 originate from PONS
- Prophylaxis = Carbamazepine
- NO Tx as short duration ?
- Confused with cluster headache if occuring in the V1 =Opthalmic branch of trigeminal nerve distribution
what part of the brain is fucked up in Trigeminal neuralgia ?
PONS
Prophylaxis of trigeminal neuralgia ?
Carbamazepine
TN is confused with which type of headache ?
Often confused with cluster headache if occurring in the V1 (ophthalmic branch of
trigeminal nerve) distribution.
Cluster headache ?
- Lancinating=Piercing sensation
- Knife like headache
- Ipsilateral lacrimation
- Rhinorrhea=Runny nose
- Pupillary changes
- NBME=====Ipsilateral mitotic Pupil
- Min—Half hour
- Longer than trigeminal neuralgia
- Prophylaxis=Verapamil
- Tx=O2
where the Q
will say the guy wakes up from sleep + paces around his room until pain eventually
goes away.
Prophylaxis of cluster headache ?
Verapamil
Ipsilateral miotic pupil in which headache ?
Cluster headache
the guy wakes up from sleep + paces around his room until pain eventually
goes away== which headache ??
Cluster headache
Migraine ?
- Unilateral throbbing /Pounding headache
- Last hour
- Prophylaxis=Propanolol
- Tx=NSAID -Triptans
- Triptan=Serotonin 5HT receptor agonists
- Prodromal visual aura or sound =Warning sign of headache
- USMLE will give 30s female with hypertension who has migraine Hx, and the treatment is “beta blockade” for her HTN management.
Estrogen-containing contraceptives are contraindicated
Pregnant women + HTN + migraine?
Beta blokade
Propanolol
Prophylaxis of migraine ?
Propanolol
Migraine association ?
prodromal visual aura or sounds
Migraine + Aura what is contraindicated ?
Estrogen containing contraceptives
Tx of migraine ?
NSAID + Triptans
Triptan = Serotonin 5HT receptor agonists `
Tension headache ?
- Bilateral
- Dull
- Band like
- Sleep
- Acetaminophen
Tx of tension headache ?
- Sleep
- Acetaminophen
Temporal arteritis ?
Giant cell arteritis
1.9/10 Qs will be painful unilateral headache in patient over 50.
2. Low grade fever
3. High ESR
4. Flares
5. Proximal Muscle pain + Stiffness =Polymyalgia rheumatica (PMR)
6. Pain with chewing=Jaw claudication
7. To prevent blindness= Give STEROIDS before biopsy
8. Or else complication = Ischemic Optic Neuropathy
9. Which steroid = Methylprednisolone
10.Day time line = 3 Days
11.Diagnostic step= Biopsy now Steroid is not included
To prevent blindness in temporal arteritis which step ?
Steroids before biopsy
If steroid is not given which complications will happen ?
Ischemic optic neuropathy
Association of temporal arteritis ?
- Polymyalgia rheumatica
- Jaw claudication
- Low ESR
- Low grade fever
- Flares
Which steroid is given before biopsy ?
Methylprednisolone
Tension type Headache ?
FA
- Bilateral Both sided
- Female common
- Band like pain
- > 30minute
- Most common primary headache
- No nausea No vomiting
- No aura
- Tx-NSAID , Acetaminophen

Migraine
FA
POUND
P=Pulsatile
O=One day
U=Unilateral
N=Nausea
D=Disabling
532
Cluster headache ?
FA
- Unilateral
- Horners syndrome
- Common in male
- Lacrimation
- Rhinorrhea
- Conjunctival infection
532
Trigeminal neuralgia which CN ?
CN-5
