*These are ONLY if absent for 2 or more occasions as listed above in a calendar year

Who arranges for reimbursement of a doctors note?

PC submits to the Assistant chief of operations and communications. *receipt must be submitted with the note

In order to qualify for Sick Leave Credits (SLC) exemptions under the attendance support management program (ASMP) agreement, staff need to provide the following documentation to employee health services in order to support the absences (4)

1. First date that treatment was sought
2. Diagnosis of condition
3. Treatment for condition
4. Physician (Doctor) note indicating date for staff to return to work
