Human Factor Flashcards
What is the goal of the maintence Error decision aid (MEDA)?
is to investigate errors, understand root causes, prevent and mange accidents. (most of the factors that contribute to an error can be managed).
What is an Active failure?
Is one in which the effects are immediate
What is an Latent Failure?
Occur as a result of a decision made or action taken long before the incident or accident actually occurs.
Whats another concept of investigating and evaluating maintenance errors?
The “SHEL” Model. The SHEL represents software, hardware, environment and liveware, with the human as the starting point
Human error is defined as what?
Human error is defined as human action with unintended consequences, also is the study of how people interact with thier envirement.
What are the magnificent Seven?
1- We worl to accentuate the positive and eliminate the negative. 2- safety is not a game because the price of losing is too high 3- just for today Zero error 4- We all do our part to prevent Murphy from hitting the jackpot 5-our signature is our word and more precious than gold 6- we are all part of the team 7- we always work with safty net.
what are the three human error definition?
Omission: not performing an act or task
Commission: accomplishing a task incorrectly
Extraneous: Performing a task not authorized.
What is the percentage of accident result of human error?
Approximately 75-80 percent
what are the consequences of human error?
1- Little or no effect 2- Damage to equipment/Hardware 3- Personal injury 4- Catastrophic.
What are the Dirty dozen?-
1- Lack of communication, 2- Complacency 3- Lack of knowledge 4- Distraction 5- Lack of Team work 6- Fatigue 7-Lack of resources 8- Pressure 9- Lack of assertiveness 10- stress 11- lack of awarness 12- Norms
List of human factor that effect AMTs (Mechanic)?
Boring repetitive jobs- Loud noise -Poor training -Fatigue -Substance abuse .
What are the element of Human Fators?
Clinical psychology 2- Exprimental psychology 3- Anthropometry 4- Computer science 5-Cognitive science 6-Safety Engineering, 7-Medical science 8- Organizational psychology 9- Educational psychology 10- Industrial Engineering
Example of common maintenance Errors
Incorect installation of components. 2- Fitting the worng parts 3- Electrical wiring discrepancies to include crossing connection 4- forgotten tool and part 5- failure to lubricate 6- faillure to secure access panels, fairing, or crowling 7- fuel or oil caps and fuel panels not secured 8- failure to remove lock pin
