HTN and Ischemic Heart Disease Flashcards
1
Q
4 CV Conditions requiring eval before surgery?
A
- ACS/Unstable Angina
- Decompensated HF
- Arrhythmias
- Severe Valvular Disease
2
Q
Do you revascularize patients prior to surgery?
A
- Decision for coronary intervention made INDEPENDENTLY from surgical procedure
- Can they be on DAPT for surgery?
3
Q
Classes of HTN
A
Stage 1 - 150/90
Stage 2 - 160/100
Stage 3 - 200/110
Stage 4 - Higher
4
Q
Chronic increases in afterload
A
results in chronic wall tension –> hypertrophy of the ventricle and diastolic dysfunction
5
Q
What causes ischemic heart disease?
A
- Marked increase in myocardial metabolic demand
2. Reduction in myocardial oxygen supply
6
Q
Conditions that cause ischemic?
A
- Atherosclerosis
- Severe HTN
- Severe tachycardia
- Hypotension, hypoxia, anemia
- Valvular Disease (AS)
7
Q
Arrhythmia most likely following ACS?
A
V-Fib!!! First 4 hours
8
Q
How does nitrate treat ACS?
A
- reduces wall tension afterload of LV
- reduces myocardial O2 consumption
- Dilates coronaries
- increases myocardial O2 supply
9
Q
Best way to treat ACS in OR?
A
Maintain the supply/demand relationship
- aggressively treat tachycardia and HTN and analgesia
- Increase FiO2, avoid hypotension
