What is the framework for providing care?
Four components:
1. gathering information
2. assessment and decision-making
3. communication of decision with patient and/or Healthcare team
4. follow up: monitor and evaluate
What are 7 types of information you can gather?
framework: gathering information
- medical history
- family history
- social history
- demographics
- presenting complaint
- medication history
- patient perspective
What does assessment and decision-making entail? What are the two frameworks associated with it?
[i] Identifying and prioritizing medicine-related problems.
[ii] [a] Framework 1:
* patient factors: health literacy, demographics, understanding, affordability, beliefs;
* disease factors: past and current history;
* medication factors: is it funded? affordability, adverse drug effects, drug interaction, how easy is it to take;
* environment/system factors: prescription errors, confusion during care transition.
[ii] [b] Framework 2:
* indication: why is drug being used? Unnecessary or additional medicine required;
* efficacy: ineffective medicine or dose too low;
* safety: side effects, dose too high, interactions;
* adherence: nonadherence or overadherence.
What two tools can you use to communication?
- ISBAR:
identify (introduce self, position, location and patient); situation (what concern is about / urgency); background (provide context and clinical information); assessment (consideration of options); recommendations (by whom and when). - SOAP Notes:
(introduce yourself); subjective information; objective information; assessment (your thought process on problem); plan (actions to be taken to address problem).
What does evaluation and monitoring entail?
[I] identify goals of therapy & adjust medication plan as necessary.
[2] monitoring may be required to ensure safety, adherence and efficacy of drug.
identify clinical or lab tests needed to monitor achievement toward goals: what is monitoring required for? (safety? efficacy? adherence?) What will be monitored? Who will do the monitoring? When will monitoring need to occur?
