Special Care Unit Procedure
Procedure is to talk directly to the primary care nurse in addition to review of medical record as status could change from hour to hour
Respiratory Equipment
- Flow Meter : “Christmas Tree” : green portion where tubing attaches.
Nasal Cannula
Mask : Standard, Non-rebreather

Endotrachial Tube = intubated
-Methods to prevent pulling out.

OT Take Home Messages for Respiratory Equipments (6)
- Know setting & Check Flow Meter
- Make sure cannula or mask is in place before activity.
- Reduce resistance or repetitions, go slow
- Facilitate communication to reduce frustration by asking for simple responses.
- Remove only one restraint at a time; supervise closely
- Corrigated tubing collects moisture from breath and makes more noise… sometimes you have to lift it and let it drain.
Ventilator Dependent Patients Generally Can Not Tolerate
Active motion overhead or resistance.
Gastro-intestinal Equipment (GI) : Nasogastric Tube
For feeding or removing stomach contents (suction)
GI Equipment : Dobhoff feeding tube
Thinner and more comfortable through the nose
GI Equipment : Gastrict Tube
Feeding directly through abdominal wall
GI Equipment : Ostomy, Colostomy, Iliostomy
Opening trough abdominal wall to remove fecal matter
Stoma covered in plastic bag to collect waste.
OT Take Home Messages for GI Equipment (3)
- Feeding tube active : keep head of bed at 30 degrees of elevation.
- Colostomy bag greater than 1/2 full : ask nurse to empty before activity
- Pay attention to tubes to prevent pulling out : notify nurse if tape to hold tubes in place is very loose.

NPO
nothing by mouth
Genito-urinary Equipment (2 types)
- Foley Catheter: Indwelling Catheter
- tube inserted into urethra to the bladder to collect urine stored in an external bag. - External Catheter: Condom style for men only.
OT Take home messages for catheters (2)
- Catheter bag should ALWAYS be blow bladder: urine could flow back into the bladder which is a risk for UTI
- Pay attention to tubing so it does not get pulled or snagged.
OT Take home messages for IV Equipment (3)
- Pumps sound alarms : Do not panic, usually means its time to replace fluids or medication. Call nurse and ask.
- Pay attention to tubes: easily caught in sheet, under body.
- Where tubes enter body there is high risk for infection, so reduce exposure, use gloves.
What should you avid at sites of IV insertion
Excess movement, ROM
IV Insertions at the skull site
Avoid neck flexion, avoid isometric
IV insertions at the wrist
Avoid heavy gripping or excess ROM
IV insertions at the subclavian site
Avoid excess shoulder ROM
Hospital Care Take Home Messages (4)
- If you dont know ask.
- Make sure you put guard rails up and restraints back on when you leave.
- Go slow if client is in fragile condition.
- Supervise closely.
