Helicopter criteria- Pedi. (1 red/ 2 blue)

A

Red: advanced airway, penetrating injury, spinal cord injury, amputated proximal wrist/ankle, flail chest, 2+ long bone fractures, pelvic fracture, open/depressed skull fracture, pulse less extremity, burns >10% TBSA
Blue:
GCS <14 trauma , GCS < 10 nontrauma, LOC> 5 minutes, resp rate > 40/minute, HR > 150 min,
BP < 70+2age

Adult Helicopter
Amputation proximal to wrist/ankle

Helicopter PEARLS
Anticipated ground transport time. Consider for unstable pedi drowning. If expected landing is 20 minutes beyond extrication then ground transport. Do not use for CPR.

Trauma Field Triage. Level 1 or 2 Center
Advanced airway. GCS <14. All blunt/penetrating trauma with unstable vitals. All penetrating injuries (or with CPR). Chest wall instability. Pelvic fractures/ two or more longbone fractures. Crush/degloved/mangled extremity. Skull fracture. Paralysis or hanging injuries. Profuse bleeding or tourniquet application.

Trauma Field Triage- Level 1,2,or 3 Trauma Center
Fall >10 feet. High risk auto crash. Auto v Ped (>5 mph impact). Motorcycle/bicycle crash > 5mph. Pregnancy > 20 weeks Older adults with risk of injury and death increases after 55. Anticoagulation and bleeding disorders, specifically w/head trauma.

Trauma Field Triage Pearls
When in doubt, transport to level 1/2. | Blunt traumatic CPR patients should be transported to closest hospital.

Medical Director Notification
Action/inaction that could lead to injury or death. Performance outside the scope of credentials/qualifications. Missing/tampered controlled substances. Any attempt of surgical airway. Sudden cardiac arrest after: antiarrhythmic in a previously hemodynamically stable pt. Physical, chemical, or mechanical restraint. Sedation. Analgesia. Neuromuscular blockade. Taser by PD.
