Hoist hydraulic overheat

A

Symptoms: HYD OVERHEAT light on

Actions:
1. As soon as practical, cease operation of the rescue hoist until light goes out.

  1. Hoist Circuit Breakers- Check/Reset
    a. Rescue Hoist Power
    b. Rescue Hoist Cont
    c. Rescue Hoist Cont

ICS Failure

A

Actions:
1. Perform ICS check, check Hoist Pendant ICS switch and visually inspect cord. If in position to safely do so, retrieve the device. (FM)

  1. Perform an ICS check, check PTT and cord. (P)
  2. Slowly move aircraft to establish visual reference with hoisting area. (P)
  3. Pay out slack - As Required (FM)
  4. Troubleshoot ICS malfunction. If corrected, continue normal hoist procedures.

If ICS malfunction is not corrected:
6. Position aircraft to recover rescue device per Rescue Briefing. (P)

  1. Signal Pilot to reestablish visual reference with hoisting area by pointing to the left side of aircraft. Rescue device may or may not have been recovered. (FM)
  2. Observe signal, pass to Pilot. (CP)
  3. Slowly move aircraft to establish visual reference with the hoisting area. (P)

If device was recovered:
10. Complete hoist of rescue device. (FM)

If device was not recovered:
11. Make onscene repairs, abort the mission, or take other action as appropriate.

Runaway Hoist

A
  1. Advise the pilot.

Caution: when BACKUP control is rapidly cycled ON/OFF, upward movement/jump of hoist cable is possible.

  1. “Going to BACKUP Control”- Push ON (FM)

If runaway continues:
3. “Set BACKUP PUMP OFF”. (FM) (CP performs)

  1. Abort the hoist and make onscene repairs, abort the mission, or take other actions as appropriate.

Fouled Cable

A

Actions:

  1. Immediately pay out cable.
  2. Advise the pilot.

If injury or danger is imminent:
1. “SHEAR, SHEAR, SHEAR”. (FM)

Warning: Shear capabilities are lost if RESCUE HOIST CONT switch is OFF.

  1. EMER REL-Press (P,CP)
  2. Hoist Shear-Shear (FM)

If injury or damage is not imminent:

  1. Maintain position.
  2. Attempt to have FM and boat personnel free cable or rescue device.

If unable to free device from deck:

  1. Cut cable with cable cutters.
  2. Abort mission or rig quick splice and continue hoist.

Hoist Failure

A
  1. Advise the Pilot.
  2. “Check BACKUP pump ON”. (FM) (CP performs)
  3. “Check RESCUE Hoist to ALL”. (FM) (CP performs)

If condition persists:

Caution: when operating in BACKUP control; Down limit switch still works, UP limit switch is inactive.

Caution: when BACKUP control is rapidly cycled, upward movement/jump of hoist cable is possible.

  1. “Going to BACKUP control”- Push ON (FM)
  2. Complete hoist or abort as required.

After hoist is complete, troubleshoot as follows:

  1. BACKUP control-Push OFF (FM)
  2. Hoist Circuit Breakers-Check/reset
    a. RESCUE Hoist Power
    b. RESCUE Hoist Cont
    c. RESCUE Hoist Cont
  3. Check operation of pendant or grip.

If condition persists:
9. Disconnect pendant/recheck grip.

Lost Swimmer Day

A
  1. Mark and Note position.
  2. Transition and establish right hand orbit.
  3. Search/Monitor for swimmer signal
    A. Radio
    B. Smoke
    C. Mirror
  4. When visual, resume hover position.

If unable to relocate:

  1. Deploy the lighted raft at the marked position.
  2. Deploy the DMB at the marked position.
  3. Request assistance.
  4. Initiate a sector search.

Lost Swimmer Night

A
  1. Mark and Note position.
  2. Flash Landing/Hover lights.
  3. Search/Monitor for swimmer signal.
    A. Strobe light
    B. Radio
    C. Flare
  4. When visual, cease flashing light and resume hover position.

If unable to relocate:

  1. Deploy the lighted raft at the marked position.
  2. Deploy the DMB at the marked position.
  3. Request assistance.
  4. Initiate a sector search.

Leaving swimmer on scene

A
  1. Mark and Note position.
  2. Deploy lighted raft.
  3. Deploy DMB.
  4. If time permits contact swimmer via radio and advise of situation/intentions.
  5. Notify the SAR mission coordinator.

Emergency flyaway during hoisting ops

A

If swimmer is attached to hoist hook between the water and cabin door:

  1. Shear, Shear, Shear
  2. Emer rel- Press (P/CP)
  3. Hoist shear switch- shear (FM)

If swimmer is between cabin door and hoist drum:
4. Bring swimmer into the cabin or maintain a firm handhold.
