Hoist failure
Hoist fails to respond to flight mechanic inputs
- Announce “hoist failure” and advise the PF “committed” or “not committed”
If committed: - Check the cable and drum for fouling
- Activate emergency hoist power and continue the hoist
If hoist power is not restored - Activate manual override and continue the hoist
If hoist fails to respond and no personnel in recur device - Retrieve the rescue device if applicable
Unable to retrieve rescue device - Cut cable with cable cutter or shear.
- Once cable is clear, announce “cable is away”
If not committed - Abort the hoist
Hoist boom failure
Hoist boom does not respond to electrical control inputs
- Announce “boom failure”
- Hoist master switch- check crew position.
- Hoist act control and hoist act power circuit breakers (2) - cycle
If boom doesn’t respond to electrical control inputs - Hoist act circuit breakers - pull
- Hoist pendant - disconnect
- Manual crank - install
- Manually crank boom to stowed position
- Manual crank- remove
Hoist electrical runaway
Hoist cable extends/retracts without crew input
- Secure hoist power at the hoist control panel ONLY
- Announce “hoist runaway” and advise pf committed or not committed
If committed - Activate manual override and continue hoist.
If not committed - Abort the hoist.
Lost Comms during hoisting operation
Pilot and/or fm unable to listen and/or talk during a hoist
- FM- tap the of and give hold signal.
- Announce “ICS failure” and advise the pf committed or not committed
If committed: - Communicate by vocal commands and continue hoist.
Once hoist is completed:
FM - ensure hot mic is selected, helmets properly connected; aatempt hot mic ics comms.
If communications are not restored: - Press push-to-talk switch- attempt normal comms.
If communication is still not restored: - Audio control panel- select ALTN and attempt comms. (All call and hot mic inop in ALTN)
Communications still not restored: - Abort mission
Not committed - Abort hoist
Fouled/damaged
- Hoist cable becomes fouled or caught on deck or ground items
- Hoist cable visibly damaged
Injury or damage is imminent:
1. Announce cable fouled.
2. Activate hoist shear switch while announcing “shear shear shear”
3. Once cable is cleared announce “cable is away”
If conditions permit:
1. Pay out slack in cable
2. Advise pilot
3. Direct the vessels crew to free the cable
4. If unable to free the cable or the cable is damaged, and situation permits, cut the cable utilizing cable cutters and announce “cable is away”
5. If mission urgency dictates a continued hoist, assemble the quick splice and complete the hoist with remaining cable length.
Lost swimmer- DAY
- Mark position
- Transition and establish right orbit.
- Look and listen for RS signal
- When visual- resume hover position
If these procedures fail to relocate RS - Complete leaving RS on scene procedure
Daytime signals include:
Radio
Smoke
Mirror
Lost swimmer -NIGHT
- Mark position
- Flash landing /hover light
- Look and listen for RS signal
- When visual- cease flashing light
If these procedures fail to locate RS: - Complete leaving rescue swimmer on scene procedure
Nighttime RS signals
Radio
Strive light
Flare
Leaving rescue swimmer on scene
- Mark and record position
- Deploy lighted raft
- Deploy DMB
- Notify opcen or other rescued resource
Emergency recovery of RS
- Rescue basket - if rigged and empty
- Bare hoist hook - if basket not readily available
- With the assistance of the FM, the RS shall enter the cabin backwards.
