Hodgkin's Lymphoma Flashcards
When does incidence of Hodgkins Lymphoma peak?
20-30
Smaller peak in old age
What is Hodgkins Lymphoma associated with?
Epstein Barr Virus and CMV
Previous severe glandular fever
What cell is the hallmark of Hodgkins Lymphoma?
Reed-Sternberg Cell
Describe the features of a Reed Sternberg Cell
Multi-nucleated Giant Malignant B-Cell Derivatives On a background of a variety of cells
What is a contiguous disease?
Only spreads to adjacent nodes and structures due to the flow of lymph
What are the types of classical Hodgkin’s lymphoma?
Nodular Sclerosing - 70%
Mixed Cellularity
Lymphocyte rich
Lymphocyte depleted
Describe Nodular Sclerosing Hodgkins Lymphoma
lacunar cells
young women
Describe mixed cellularity Hodgkins Lymphoma
+++Reed-Sternberg
lymphocytes, eosinophils, neutrophils, and histiocytes
no fibrotic bands
elderly
Describe lymphocyte rich Hodgkins Lymphoma
small lymphocytes
good prognosis
Describe lymphocyte depleted Hodgkins Lymphoma
associated with HIV
popcorn cells
worst prognosis
What are popcorn cells?
a variant of Reed Sternberg cells also seen in Nodular Lymphocyte Predominant HL
Of B cell lineage and express CD20
Also known as L&H hodgkin cells
What is Nodular Lymphocyte Predominant Hodgkins Lymphoma?
A non classical HL which has popcorn cells rather than Reed Sternberg cells
Has a different management
How does Hodgkins Lymphoma present?
Painless rubbery lymphadenopathy - esp. cervical
B Symptoms
Itch
Alcohol Induced Lymph node pain
Chest discomfort with cough/SOB - mediastinal involvement
Describe B symptoms of Hodgkins Lymphoma
Fever
Night sweats
>10% weight loss in 6months
How is Hodgkins Lymphoma investigated?
Excisional node biopsy - definitive diagnosis
CT neck chest abdo pelvis - staging
FBC -normocytic anaemia, thrombocytopenia
Lactate dehydrogenase - prognosis
ESR- prognosis
HIV test
