Hodgkin's Lymphoma Flashcards

1
Q

Hodgkin’s Lymphoma Symptoms

A
  • Painless adenopathy which can fluctuate
  • “B symptoms”

*fever

*night sweats

*weight loss

*generalized pruritus w/o rash- due to inflammatory cells (increased eosinophils and mast cells)

  • Nodal pain w/ ethanol ingestion
  • Spread generally predictable- cervical nodes downward

Q

Hodgkin Lymphoma Diagnosis w/ Flow Cytometry

A
  • Flow cytometry is not useful for HL b/c neoplastic cells are in the minority, as opposed to in NHL where the majority of cells are neoplastic

Q

How Does HL form?

A
  • Activation of transcription factor NF-kB

*EBV infection- turn on genes that promote lymphocyte survival and proliferation

*EBV+ tumor cells express latent membrane protein (LMP-1) that upregulates NF-kB

  • Loss of function mutations
  • Rescues damaged germinal center B-lymphocytes that cannot express Igs from apoptosis => acquisition of mutations => Reed-Sternberg cells

Q

Reed-Sternberg Cells

A
  • “Crippled” germina center B cells, meaning they have not undergone hypermutation to express their antibody
  • Stained by CD30 staining
