What are risk factors for HIV/AIDS?
Needle sharing, unprotected sex, needle stick injury
How does HIV enter cells?
It binds to CD4 receptors via GP120 envelope glycoproteins
How does primary HIV infection present?
Flu-like symptoms and a rash 2-4 weeks after infection, Can have persistent generalised lymphadenopathy
What test is gold standard?
ELISA for HIV antibody and antigen
What tests are used to monitor progression?
Viral load, CD4 count
What test is used to test for vertical transmission?
Nucleic acid testing/viral PCR
What are some differential diagnoses?
Infectious mononucleosis, influenza, cytomegalovirus infection
What do CCR5 antagonists do?
Inhibit entry of virus into cells
What types of reverse transcriptase inhibitors are there?
Nucleoside reverse transcriptase inhibitors and non-nucleoside reverse transcriptase inhibitors
What other enzyme inhibitors can be used?
Integrase strand transfer inhibitors, protease inhibitors
What can lower the dose needed?
Pharmacokinetic enhancers
What are the steps done before starting drugs?
Counselling, screen for infections and malignancy, baseline tests, review usual medications for potential drug interactions
What are the first line drugs in the UK?
NRTI, protease inhibitors, NNRTI, integrase inhibitor
What should you monitor?
Adherence, adverse effects, CD4 count
What is the commonest systemic fungal infection in HIV?
Cryptococcus neoformans
