Abacavir

Abacavir BBW
- Screen for the HLA-B*5701 allele before starting (+ is contraindicated)
- Hypersensitivity reactions (fever, rash, fatigue, malaise, GI, N/V/D, ab pain)
- Respiratory symptoms (dyspnea cough)
Abacavir contraindications
- Hypersensitivity to abacavir
- moderate to severe hepatic impairment
Abacavir Warnings
- caution in CVD due to possible increase risk of MI, Lactic acidosis and severe hepatomegaly with steatosis
Abacavir SE
N/V, HA, Increase LFT, hyperlipidemia
Abacavir Notes
-caution with alcohol
-Warning card (summarizing symptoms of hypersensitivity required)
-ABC/lamivudine with either efavirenz or atazanavir + ritonavir should not be used in patients with pre treatment HIV viral load > 100,000 copies/ml

Abbreviation: 3TC
Brand: Epivir
Class: NRTI
Lamivudine BBW
-Do not use epivir-HBV for TXT of HIV (contains lower dose of lamivudine)
-can result in HIV resistance
-severe exacerbation of Hep B (HBV) can occur when drug is discontinued in patients with HBV
Lamivudine Warnings
-lactic acidosis
-severe hepatomegaly with steatosis
Lamivudine SE
HA, N/V/D, fatigue, insomnia, myalgias, increase LFT
Lamivudine monitoring
LFT, renal function, HBV status prior to initiation
Lamivudine Notes
- avoid combining with emtricitabine (antagonists interaction as both are cytosine analogs
-Has activity against HBV
-In HIV/HBV co infection must dose lamivudine at the higher HIV TXT dose found in Epivir and symfi (epivir =/= epivir HBV)

severe exacerbation of Hep B (HBV) can occur when drug is discontinued in patients with HBV
-Truvada for pre exposure ppx only for patients confirmed to be HIV negative prior to initiating and Q3 months
Emtricitabine warnings
-lactic acidosis
- hepatomegaly with steatosis

N/V/D, rash, dizziness, HA, insomnia, hyperpigmentation in palmes and soles
- increase CPK and LFT
