What is Quality?
This describes the character of the symptoms; color and consistency of bodily fluids, size of lumps or lesions, type of cough, acute vs. chronic, use specific descriptors
What is Severity?
This describes the intensity, quantity, or extensiveness of the issue; intensity of pain; pain scale 1-10; number of lesions or lumps
What does Onset mean?
Involves the patient explaining what he/she was doing when the problem began

The circumstances that produce a problem or intensify it; should be well defined; ex: pain worse with exercise
