Hip Fractures Flashcards
2 classifications of hip fractures
Intracapsular (proximal to intertrochanteric line)
Extracapsular (distal to intertrochanteric line)
2 types of intracapsular fracture
Sub-capital
Transcervical
Both can also be displaced or un-displaced
What artery is at risk from intracapsular fracture and what will this cause
Medial femoral circumflex artery -> ANV of femoral head and non-union
What classification system is used for intracapsular fractures
Garden Classification types I-IV
What is type I garden fracture
Incomplete fracture/ valgus impacted
What is type II garden fracture
Complete fracture NO displacement
What is type III garden fracture
Complete fracture with partial displacement
What is type IV garden fracture
Complete fracture with total displacement
What are the 4 types of extracapsular fracture
Intertrochanteric
Subtrochanteric
Basi cervical
Reverse oblique
What investigations are completed for hip fractures
- X-ray ‘Gold standard’, identifies most fractures
- MRI if fracture invisible on AP & lateral X-ray as undisplaced
Surgical management of intracapsular high functioning individual fractures
If displaced = total hip replacement
If un-displaced = Cannulated screw fixation
Surgical management of intracapsular low functioning individual fractures
Hemiarthroplasty
Surgical management of extracapsular intertrochanteric fracture
Dynamic hip screw
