Hip muscle strain

A
  • Activity or trauma related with hyperextension, mostly groin
  • Pt can tell exactly how and when it happened
    S&S:
  • Pain increases with activity
  • Swelling, bruising, localized tenderness.
  • Pain and weakness with active ROM
    DX:
    -Radiograph only if fracture
    TX:
  • Pain mgmt
  • Crutches
  • PT

    S&S:
  • Increased activity intolerance- running
  • Snapping or clunking at hip flexor crease with tendinitis
    DX:
  • MRI
    TX:
  • Pain mgmt
  • Crutches

Slipped capital femoral epiphysis (SCFE)

  • Dislocation of head of femur at epiphyseal plate
  • Common in adolescent boys
    S&S:
  • New onset of limp with inability to bear weight
  • Decreased abduction and internal rotation
    DX:
    -XRay
    TX:
  • Ortho referral

Developmental dysplasia of the hip DDD

A
  • Congenital
  • Newborns born breech- mostly girls
    S&S:
  • Leg discrepancy and asymmetrical gluteal folds
    DX:
  • Barlow and Ortolini: First Barlow: dislocate the hip by adducting then Ortolini to confirm dysplasia by abducting and hearing a clunk
    TX:
  • Ortho
