Mechanism of Action
Blocks the conversion of prothrombin to thrombin and prevents the conversion of fibrinogen to fibrin
Indications
Acute myocardial infarction
Prophylaxis and treatment of thromboembolic disorders (DVT , PE)
Contraindications
Hypersensitivity
Active bleeding
Recent intracranial, intraspinal , eye surgery
Severe Hypertension
Side effects / adverse reactions
Hemorrhage
Pain
Bruising
Hematuria
Gi bleeding
Shock
Thrombocytopenia
Adult dose
Bolus 60 U/KG max 4,000 U IV/SQ
Infusion 12-15 U/KG HR max 1,000 U/HR
Pediatric dose
Not recommended
Drug Interactions
Chance of bleeding increases when used in conjunction with other anticoagulants
Duration of Action
Onset - immediate IV 20-60 minutes SQ
Peak - variable
Duration - 4-8 hours
