What are the triggers of Henoch-Schonlein Purpura ?
MAFIC (almost the same as for LCV MANIC, but replace NSAIDS with Food)
- Medications
- autoimmune condtions
- foods (blue cheese)
- infections
- cancer
What is classic tetrad of Henoch-Schonlein purpura?
- palpable purpura of legs and butt
- arthralgia of knees and ankles
- GI issues (abdominal pain and diarrhea w/ or w/o bleeding
- renal changes: hematuria, possible nephritis, renal failure in 1% of cases
What percent of cases of Henoch-Schonlein purpura can have renal failure?
about 1%
What types of cancers are a/w Henoch-Schonlein purpura?
solid organ and hematologic
What are the risk factors for renal involvement with Henoch-Schonlein purpura in adults?
- purpura above the waist (“closer to kidneys”)
- fevers
- elevated ESR
Histology and DIF of Henoch-Schonlein purpura:
histology is same as LCV (because it is a small vessel vasculitis)
- DIF with IgA in blood vessel walls
Treatment for Henoch-Schonlein purpura:
supportive
- can use prednisone or dapsone
- monitor with serial UA’s and stool guac if GI symptoms
Serial lab monitoring for patients with Henoch-Schonlein purpura includes:
serial UA’s and stool guiacs (if GI involvement)
