Essential Dx for a hemothorax
Decreased breath sounds
Dullness to percussion ( affected side)
Respiratory distress
Hypotension
MOI leading to a hemothorax
Injury to chest wall, great vessels, or lung
Hemothorax is secondary to?
Penetrating injury disrupting pulmonary or systemic blood vessels
In great vessel injuries what percentage die immediately?
50%
25% live for either?
5-10 minutes
30 minutes or longer
Physical findings
Respiratory distress
Tachypnea
Variable degrees of hypoxia
Dullness to percussion, decreased breath sounds on affected side
Hypotension and flattened neck veins
Pulse pressure narrow
Diagnosed confirmed with?
Chest X-ray
What volume of blood is seen in in xray
200-300ml
What can help to evaluate for hemothorax at bedside quickly
Ultrasound
Treatment?
Airway intact
Correct hypoxia
Provide suction ans intubation if airway not intact
Tube thoracotomy w/ 36 or 40 french chest tube
