When is hematuria significant?

A

> 25 RBC/HPF = worrisome

Both gross and microscopic hematuria are significant

Note: 85% bladder cancer cases present with hematuria

  • Malignant neoplasms (renal & bladder)
  • Infection
  • Renal calculi
  • Coagulopathy
  • Glomerular disease
  • Hydronephrosis
  • Trauma
  • Polycystic kidneys
  • Medication
  • BPH
  • Exercise-induced (resolves in 72 hours)

List risk factors for hematuria-related malignancy.

A
  • Chronic UTI’s
  • History of pelvic irradiation
  • Age > 40 years***
  • Smoking history
  • Excessive acetaminophen/NSAIDs
  • Men > women
  • Occupational exposure

A

Consider referral to urology if:
- Age > 40 years
- Other risk factors for malignancy present
