Heat Stroke Flashcards
5 Main steps to Rx heat stroke?
- IV catheter ; Oxygen
- Full labwork including blue top
- Cooling down
- Rx hypotension if present
- IV fluids
When to check coagulation times or also get blood in blue top?
If VERY HOT
Among all the labwork parameters, which are the most important initially?
PCV/TP
Glucose
How to cool down the pet?
- Wet down completely with COOL or COLD tap water
- Use FAN nearby
- +/- Cool water enemas
- if >109 F → intraperitoneal fluids
Why not to cover with wet towel?
It acts as a blanket and keeps them warm
When to consider giving fluids intraperitoneally?
If temp > 109 F
How much fluids to give intraperitoneally and when to remove them?
20-30 ml/kg of COOL STERILE ISOTONIC fluid
Remove after 5-10 min
When to stop aggressive cooling efforts?
When temperature reaches 103.5 F
What to do if pet is hypotensive?
-10-30 ml/kg of boluses until full shock dose is given
What to do if volume resuscitation is not effective in treating hypotension?
Consider pressor such as NER
Make sure that pet has received adequate volume first
What to do if severe hypotension, despite fluid resuscitation & NER?
Consider Dex SP 0.1 mg/kg IV once !!!
Pet needs to be on IV fluids, after cooling down and after fluid resuscitation . T/F
T
Monitoring of physical parameters?
TPR,BP,UO,SpO2 q4-6 h
Monitoring of lab parameters?
PCV/TP, Renal values & Electrolytes q12-24h
In severe cases, PT,PTT & platelet count q12h for 36 h
Why do we check PT,PTT,Platelets q12h for 36h in severe cases?
As DIC can emerge in this window
