Treatment for Stage A HF?
Lifestyle modification and ACEIs (Captopril, Enalapril, Lisinopril, Ramipril). Reduce cardiac remodeling.
Treatment for Stage B HF?
ACEIs (Captoprils, Enalapril, Lisinopril, Ramipril) or ARBs (Losartan, Candesartan, Irbesartan) and B-Blockers ( Bisoprolol, Carvedilol, Metoprolo) Rate control and decrease preload.
Treatment for Stage C/D HF?
ACEI or ARB to decrease cardiac remodeling. B-Blocker for rate control and decrease preload. Diuretic ( Chlorthalidone, Indapamide, Hydrochlorothalazide, Furosemide, Spironolactone)
Treatment for African Americans that cannot use ACEI or ARB?
Hydralazine & Isosorbide
Why can’t CCB be used in heart failure? What is the one CCB that can be used?
Decrease inotropic action. Amlodipine.
