Heart block
Delay of electrical impulse from AV node
It is due to ischemia (scar/necrotic tissue in heart), and drugs
1st Degree
PR interval > 0.20 seconds
Symptoms of 1st degree
Asymptomatic because C.O. is adequate
Causes of 1st degree
Injury/infarction of AV node
Drug effects
Treatment of 1st degree
Monitor pt
2nd degree type I
PR interval keeps elongating, then it drops QRS
Symptoms of 2nd degree type I
Asymptomatic because C.O. is adequate
May cause occasional dizziness
Causes of 2nd degree type I
Associated with acute MI Drug toxicity (digoxin, BB, CCB)
Treatment of 2nd degree type I
Remove stimulus
Monitor pt
Transcutaneous pacemaker
2nd degree type II
Normal PR interval, dropped QRS (at determined ratio)
Symptoms of 2nd degree type II
Decreased C.O.
- Dizziness/lightheaded
- Fatigue
- Fainting
- Chest pain
Causes of 2nd degree type II
Frequently associated with anterior wall MI
Requires immediate attention because it can progress to a complete heart block
Treatment of 2nd degree type II
Pacemaker
3rd degree
Complete heart block of communication between atrium and ventricle
“Divorced rhythm”
Wide QRS
Symptoms of 3rd degree
Decreased C.O.
- Dizziness/lightheaded
- Fatigue
- Fainting
- Chest pain
