What is otosclerosis ?
A
- It is an autosomal dominant condition (look for fam history)
- It is basically whee vascular spongy bone replaces lamellar bone of the bony labyrinth particularly around the oval window which fixes the stapes footplate

What are the symptoms of otosclerosis ?
A
- Conductive hearing loss
- Tinnitus
- Vertigo

What is presbycusis ?
A
- Age related hearing loss
- Loss of hearing of high pitched sounds first

Give some examples of the ototoxic drugs which can damage the ear (can cause tinnitus)
A
- Streptomycin
- Vancomycin
- Gentamicin
- chloroquine
- hydroxychloroquine
- Furosemide
- Aspirin & other NSAID’s
- Quinine

What test should be done in all patients with unilateral tinnitus/deafness?
A
MRI - to exclude acoustic neuroma

Causes of conductive hearing loss
A
- Fluid - e.g. effusion, blood, CSF
- TM perforation
- Ossicular disruption/fixation e.g. otosclerosis

Causes of sensorineural hearing loss
A
- Sensory - chochlea damage
- Neural - 8th CN (vestibulochochlear) damage

If a patient presents with sudden onset SNHL what is it important to do ?
A
Urgent referral to ENT

What is the main cause of sudden onset SNHL ?
A
Idiopathic (still requires urgent ENT referral tho)

What treatment is given to someone with sudden onset SNHL ?
A
High-dose corticosteroids
