- complete
- accurate

What if you need to make a correction on an intrastate document?
Corrections, strikethroughs and write-overs must be clearly marked, initialed and dated.
What do you do with any blanks or empty blocks on the form?
Strikeout any blocks or blank areas not used
OCVI information requirements
- Name and address of consigner (owner)
- Name and address of consignee (purchaser)
- Individual animal identification
- Description of animal
- Test date and result
- Laboratory conducting the test (if applicable)
OCVIs MUST be the original unless:
- seller/veterinary copies of completed certain for INTRAstate sale of dog or cat
- shipment of animals requiring multiple loads
- all animals individually identified on the original OCVI
- same farm of origin or assembly point
- all animals are consigned to a single destination
Who gets each color?
White-> Tallahassee (State Vet Copy)
Goldenrod-> Vet office
Pink-> Client/shipper
What information required for a Coggins Test
Owner location and address
Horse location and address (if different from owner)
Drawing or photos of horse markings (right, left, and a straight on head) PLUS written description of markings
- exception: foal under 6 months and accompanied by the mare
Coggins Test - what is needed for change of ownership? Show?
Original or certified laboratory copy required for change of ownership
Notarized copy may be used for exhibitions, shows, trail rides etc
Requirements for Interstate shipment/export of animals
1) USDA/APHIS requirements
2) Code of Federal Regulations (9 CFR) requirements
3) Specific requirements by state or country of destination
