I’m going to inspect the patient’s skin for (RBBL) rashes, bleeding, bruising, and lesions in the upper extremities and lower extremities.

Their skin is clear.

I’m going to palpate the skin for (TTM) temperature, texture, and moisture using the back of my hands in the upper extremities and lower extremities.

Their skin is WARM, DRY, and SMOOTH.

I’m going to assess for skin turgor.

There’s no tenting at the clavicle.

I’m going to inspect the nail conditon of both hands.

The nails are a CONVEX shape, and the cuticles are PINK.

Now I’m going to palpate the nails for texture.

The nails are FIRM and SMOOTH.

I’m going to inspect the hair and scalp.

The hair is evenly distributed and there are no evidence of (LI) lesions or infestations.
