lymph nodes
PRIORITY ASSESSMENT
- acute head and neck injury/pain
- acute change in vision
- acute change in abnormal vitals
- neuro changes
Subjective data
- headaches/pain
- past injuries/surgeries
- medication reconciliation
- FH of thyroid disorders
Common problems
- headaches
- neck pain
- limited neck movement
- lumps/masses
- hypo/hyperthyroidism
Normal findings of head
symmetric, round, proportional, no tenderness
Normal findings of eyes
no edema, no bulging, lens clear, sclera white
Normal findings of ears
symmetric, equal in size, no lumps
Normal findings of neck
trachea is midline, cervical spine straight, full ROM
Physical exam
- inspect/palpate scalp
(symmetry, masses, lesions) - inspect eye
(redness, drainage, trauma, asymmetries) - inspect/palpate ear
(redness, drainage, trauma, lumps, masses, tenderness) - inspection of neck
(alignment, lesions, limited ROM)
Signs of Thyroid storm
- tachycardia
- tachypnea
- n/v, diarrhea
- abdominal pain
- fever
- weakness
- hyperkinesis (abnormal excessive muscular activity, impulsive, irritable)
