HAZOP Flashcards
Definition of HAZOP
the application of a formal, systematic, critical examination of the process and engineering intentions of new facilities to assess the hazard potential of mal-operation of individual items of equipment and the consequential effects on the facility on a whole
Hazard analysis
- identification of undesired events which lead to the materialisation of a hazard
- analysis of mechanisms by which these undesired events could occur and estimation of the magnitude and the likelihood of any harmful events
HAZOP features
- aims to stimulate the imagination of designers and operators in a systematic manner so that they can identify the causes of potential hazards in the design
- flexible
- applicable to wide range of industrial installations
- should be used when modifications are made to the original system
HAZOP methodology
- performed by team of experts
- systematic consideration of each plant item
- uses guideworkds to determine consequences outside of design intent
Define intention
how the part is expected to function
Define deviation
departures from the system design which are discovered by systematic application of the guide words
Define causes
reasons why deviations occur
Define consequences
the results of the deviations
Define hazards
situations which can cause damage, injury or loss
NO/NOT
- parameter is zero
- something does not happen but no other effect
MORE THAN/LESS THAN
- increases or decreases in process parameter
- process parameter not normal
AS WELL AS
- qualitative increase
- some additional effect
PART OF
- qualitative increase
- partial effect
REVERSE
- opposite
- reverse material or flow
OTHER THAN
- substitution
- totally different effect
