What is FMEA?
A
- systematic approach and thought process used to:
- assess how a given system or process can fail and identify its potential effects
- identify corrective actions to reduce or eliminate the failure

- to identify reliability/safety critical areas/components in a system to determine where modifications may be required in the system

A
- anaylsis of the product and how failure modes affect the product operation

A
- analysis of the process by which the product is built, maintained and used
- It examines the failures in the manufacturing(or servicing) process can affect product operation

- functional
- hardware

Functional approach to FMEA
A
- coarse system breakdown
- sub-assemblies are considered which provide some required function in the system. Consider effects of loss of inputs and internal failures
- used in the early stages

A
- detailed system breakdown
- considers each individual component of the system separately
- used in detailed design

- Define system
- Construct hierarchal breakdown and functional block diagrams
- Record assumptions
- Define system failure modes
- List component(sub-system) failure modes
- Complete FMEA worksheets
- Review worksheets to determine the reliability critical components
- Make recommendations for design improvements
