A
degenerative arthritis of the 1st MTPJ
Aetiology:
- idiopathic
- post traumatic
- inflammatory
- hypermobility
- osteochondritis dissecans
NON-OP
- footwear modification - stiff soles/ rocker bottom sole
- injections
OPERATIVE
joint preserving
- cheilectomy
- 30% dorsal oblique resection
- motion preserving
- more symptoms if severe oa
- closing wedge osteotomy of PP - moberg
- dorsiflexion osteotomy - change arc of motion - adolescents - combines with cheilectomy
OPERATIVE joint sacrificing
- MTPJ fusion
- keller’s arthroplasty -low demand - transfer metatarsalgia, cock up deformity & instability - revision to fusion requires bone block
