What are some clinical clues to haemochromatosis on examination
A
Jaundice - in 75% patients
Cutaneous pigmenttion - 90% - can be an early sign
- More evident in sun exposed areas
- Can be bronze to grey
Koilonychia - 50% patients
Gynocomastia
- due to cirrhosis or hypogonadism from Fe deposition in the pituitary (this is the most common pituitary deficit)
- Amenorrhoea (less common then hypogonadism in men)
- Pubic hair loss in 60%
Hepatomegaly - common even though only 15% get cirrhosis
Hepatic bruit signifies development of HCC
Arthropathy - due to calcium pyrophosphate deposition (pseudogout) - MCP and PIP joints most affected
- In the exam look for joint replacements
Finger prick testing scars from diabetes
Evidence of cardiomyopathy
