Important points of gynaecological hx and exam
In what order should the questions be asked in a gynaecological hx?
Start with those most relevant to the presenting complaint
List the things we would like to know about menstruation
- LMP
- Cycle length and regularity
- Length of menses (<8 days)
- Heaviness
- Intermenstrual bleeding
- Post coital bleeding
- Postmenopausal bleeding
List the things we would like to know about sex/contraception
- Are they sexually active
- Dyspareunia - at penetration or deep
- Contraception
- Hx of STI’s
List the things we would like to know about cervical smears
- Last smear
2. Ever any abnormal smears and treatment
List what we would like to know about urinary symptoms/prolapse
- Frequency
- Urgency
- Incontinence
- Dysuria
- Lump at interoitus
- Heaviness or dragging sensation in vagina
What do we need to ask about the past obstetric history?
List and give details of any pregnancies and miscarriages
What is important in the family hx?
- Breast and ovarian cancer
- DM
- VTE
- Heart disease or HTN
List the steps of the gynaecological exam
- General (+/- breast)
- Abdomen
- Cervical/vaginal palpation - speculum
- Pelvic palpation - digital
In examination of the abdomen, what indicates that a mass arises from the pelvis?
If you can’t get below the mass it arises from the pelvis
What are we looking for on inspection of the external genitalia?
Vulva and vaginal oriface - ulcers, lumps, ?prolapse
What is Cusco’s speculum used for?
Inspection of the cervix and vaginal walls; can also take a smear
What is Sim’s speculum used for?
Inspection of the vaginal walls mainly for prolapse and rarely fistulae
What are we looking for on bimanual exam?
- Assess vaginal walls
- Locate cervix and assess size, shape and consistency
- Gently move cervix from side to side to assess for any tenderness (cervical excitation)
- Assess uterine size, consistency, regularity, mobility and position (anteverted/retroverted)
- Examine the adnexal structures - ovaries, masses and tenderness
