Intact cremasteric reflex
+/- dysuria, frequency
Acute epidydimitis
Severe testicular pain after prodrome (fever myalgia etc) and parotitis
Mumps
Most common bug of epididymitis in age <35
STIs (chlamydia, gonorrhea)
Most common bug in epididymitis in age >35
Ascending coliform bacteria (E coli) from bladder outlet obstruction
Pyelonephritis
Urinalysis pH >8
Renal calculus
Urease-producing organism: Proteus mirabilis or Klebsiella pneumoniae
<12hr after initial tx of early syphilis
Fever, headache, myalgia, rigors, sweating, hypotension, worsening of syphilitic rash
Jarisch-Herxheimer reaction (due to rapid destruction of spirochetes)
Resolves within 48hr
Oyster ingestion or wound infection
Increased risk in those w/ liver disease
Rapidly progressive (<12hr): septicemia, cellulitis
Vibrio vulnificus (gran neg in marine environments) -> IV ceftriaxone and doxycycline
