Indications for GTN spray
A
- Myocardial ischaemia.
- Cardiogenic pulmonary oedema.
- Control of hypertension associated with autonomic dysreflexia.
- Control of hypertension prior to fibrinolytic treatment for STEMI.
- Control of hypertension during inter-hospital transfer for STEMI.
- STEMI

Contraindications for GTN spray
A
- Known severe allergy.
- Systolic BP less than 110
- Heart rate less than 40
- Heart rate greater than 150
- Ventricular tachycardia.

Cautions for GTN spray
A
- STEMI
- Frail PTs
- Signs of shock
- Dysrhythmia
- Taken Phosphodiesterase inhibitor in last 24hr
- Known aortic/mitral stenosis

Actions of GTN spray
A
- Is a vasodilator, reduces pre load and after load of the heart therefore reducing venous return, resulting in a reduction to cardiac output

GTN dosage
A
- MI = 0.4mg (1 spray) every 5 mins, increase if caution
- STEMI = 0.4mg (1 spray). with hold if poor perfusion
- CPO = 0.8mg (2 sprays) every 5 mins, increase if caution
- Control of hypertension: 0.4 mg every five minutes.

GTN side effects/onset time
A
side effects = Hypotensive, headache, flushed, Tachycardic
onset time = 1-2mins
