What is endochondral ossification?
Bone develops from a cartilaginous precursor
Seen in the long bones of the limbs and the cranial base
What is intramembranous ossification?
Bone is formed by osteoblasts present in the mesenchymal tissue
Takes place in the mandible, maxilla and cranial vault
What is surface remodelling?
A process of resorption and deposition of bone allows the size and shape of the facial bones to change
What is primary displacement?
Caused by the bone itself changing in size
What is secondary displacement?
Growth of the adjacent bone such as in the change in the position of the maxilla due to the growth of the cranial base
What are some craniofacial growth theories?
Sutural theory (Sicher)- this theory proposes that sutures have an innate growth potential, this has been disproved by transplanting a suture- no further growth occurred Cartilaginous theory (Scott)- proposes cartilage as the primary determinant of growth Functional matrix theory (Moss)- genetic control influences soft tissue development, this in turn determines the size an shape of the bone
What are some orthodontic implications of growth?
The majority of orthodontic treatment is carried out on individuals who are still growing
In most patients 85% of their facial growth is complete but considerable growth may occur for 3-4 years after orthodontic treatment is finished
What percentage of orthodontic cases have a growth problem?

What orthodontic cases have a growth problem?
Severe skeletal class II (insufficient growth of the mandible) Severe skeletal class III (excess growth of the mandible) Severe skeletal open bites (excess vertical growth)

It is possible to influence the growth of the maxilla using reverse pull (protraction) headgear in class III cases
How can the growth of the mandible be influence?
Functional appliances may produce favourable growth of the mandible in some cases and may restrict the maxilla slightly but the majority of the effect is dentoalveolar
When might an earlier start to treatment be advantageous?
In class II discrepancies
