Graves Disease Flashcards

1
Q

What is Grave’s disease?

A
  • the most common cause of hyperthyroidism due to an autoimmune disease that is associated with an increased circulation of thyroid auto-antibodies leading to increased thyroid hormones
  • It is most common in women

Q

What is the aetiology of Graves disease?

A

1.Genetic predisposition:
50% of patients have a family history of autoimmune disease(pernicious anaemia, Hashimotos disease)
HLA-DR3, HLA-B8 alleles
2. Triggers: pregnancy, stress(psychological or surgery, trauma)

What are the clinical features of Graves disease?

A
  1. Symptoms of hyperthyroidism
  2. Triad of Graves disease
    - ophthalmopathy(exophthalmos, ocular motility disturbances, lid retraction and conjunctival problems
    - pretibial myxedema(non pitting oedema with firm plaques on the anterior or lateral aspect of the legs)

Q

How do we diagnose graves disease?

A
  1. We diagnose it using thyroid scintigraphy which shows how the iodine is being taken up in the thyroid
  2. Best initial test
  3. Thyroid antibodies

Q

What is the treatment of graves disease?

A
  1. anti-thyroid drugs like methimazole, propylthiouracil
    - mostly useful in patients that have mild disease
    - major side effects: hepatitis, agranulocytosis
    - minor side effects: rash, arthralgia
  2. B-blockers for symptomatic treatment
  3. radioactive iodine ablation
  4. thyroidectomy

Q

What is thyroid storm? or thyrotoxic crisis

A
  • pyrexia
  • tachycardia
  • delirium
17
Q

What are the complication of Graves’ disease?

A
  1. Atrial fibrillation with increased cardiac output

2. Decreased bone density

18
Q

What is the pathophysiology of Graves’ disease?

A

It is a autoimmune disorder where the production of thyroid stimulating immuniloglobulins leads to the increase of free T3/T4

19
Q

What is the function of T3/4?

A
  • Helps us burn calories
  • Stimulates our sympathetic nervous system
  • Increases heart rate and body temperature
