What is gout?
Abnormal deposition of uric acid in the body
Types of gout
Visceral and articular gout
Visceral gout
Occur due to rapidly progressing renal failure or as a terminal event in acute decompensation of ckd.
Mostly affect : pericardium peritonium and liver capsule
Microscopically urate crystals appear______
Feather crystals : basophilic spherical masses
Infectious cause of VG
IB, Avian nephritis virus, cryptosporidiosis
Non infectious cause of VG
Dehydration, vit A deficiet, urolithiasis, high Ca diet to non layers
Articular gout
Long term increase in serum uric acid level. Birds fed excessive protein.
Chronic granulomatous reaction
Synovial membrane, toes, wing joint
TOPHI
Urate crystals
Diagnosis of articular gout
Is by identifying gout
tophi-whitish yellow, subcutaneous, and
intra articular deposits that demonstrate uric
acid crystals on staining.
Treatment of articular gout
Fluid therapy
Analgesics
Allopurinol 10-30mg/kg/day PO
Colchicine 0.04mg/kg/day PO OD/BID
W3 Fatty acid 0.1-0.22mg/kg/day PO
Why surgery not preferred in AG?
Tophis are highly vascularized hence ther will be chance of severe haemorrhage.
