What is the main cause of Gout?

A

Hyperuricaemia due to impaired renal excretion of Urate

A

Over-production of Urate in the body or too much dietary intake e.g Red meat, seafood etc

Does it affect all joints?

A

It can do but it usually only affects one joint at a time

Who is affected?

A

Normally affects males (4:1), and usually occurs between 30-60 years old. Asians are more at risk

A

Male, meat, seafood, alcohol, diuretics, obesity, Hypertension, Cardiovascular Disease, raised triglycerides

Symptoms (9)

A

Acute joint pain, swelling, erythema, crescendo over 6-12 hours, fever, malaise, often has spontaneous resolution 5-15 days, itching, desquamation of overlying skin

Treatment (3)

A

High dose NSAIDs or Paracetamol, Colchicine 0.5mg/6 hours, Corticosteroids

Prophylaxis (3)

A

Allopurinol (Xanthine Oxidase Inhibitor), Lifestyle modification, manage risk factors e.g Change drugs, control Hypertension and renal function

Complications (5)

A

Renal disease, Severe degenerative Arthritis, Secondary Infection, Recurrent Painful episodes, Nerve/spinal cord impingement
