Allopurinol

A

Hyperuricemia drug - Xanthine oxidase inhibitor
1ary and 2ary gout, give w/ colchicine prophy b/c can ppt attack
Side effects: vasuculitis, agranulocytosis, hypersensitivity
DI: AlOH dec absorption, inc effect of mercaptopurines, cyclophosphamide. Inhibits elimination: chlorpropamide, warfarin, probenecib. Dec effect of fluorouracil.

Probenecib

A

Hyperuricemia drug - Uricosuric agent
Risk of kidney stones, keep urine pH 6.0 (give alkali) and high vol
Give w/ prophylaxis colchicine b/c can ppt attack (start 2wks after)
Side effect: GI irritation blocks uric acid reabs- inc excretion
Lots of DI- decreases excretion of acidic cmpds!!!

A

Rasburicase

A

Hyperuricemia drug -Enzyme converting uric acid to allantoin
Recombinant form of urate oxidase
IV
SE: hypersensitivity (not a human enzyme) can only use once

Colchicine

A

For acute gouty attack- No analgesic effect!!!
Binds tubulin, inhibits leukocyte migration and phago (anti-inflammatory). Inhibits LT B4
effective in 12-24hr
Adverse: diarrhea
