A

Insuficiencia renal, HTA, obesidad, acidosis sistémica, fármacos (tiazidas, OH, aspirina)

Gota aguda intermitente

A

Dolor intenso una o dos articulaciones + tumefacción, eritema, calor local.
Más afectada: 1° MTF (podagra)
Otras: tobillo, medio pie y rodilla
Sint sistemicos: fiebre, calosfríos, CEG
Ataques agudos autolimitados

Gota crónica

A

Poliarticular crónica. Varios años desde la gota aguda intermitente.
Tofos subcutáneos: muñecas, dedos, oído externo, rodillas, bursa olecraneana, tendón de aquiles
Dx dif: AR

Examen microscópico de líquido sinovial con luz polarizada → cristales con birrefringencia (-)

Corregir sobrepeso, cambiar hábitos

A

Reposo
Colchicina oral/AINEs
2° línea corticoides
No iniciar alopurinol en esta etapa (si paciente tomaba, dejarlo)
