What is GORD?
Symptoms due to reflux of stomach contents (acid/bile)
Common presentations of GORD?
Retrosternal chest pain (‘heartburn’), belching (burp), dyspepsia (indigestion), nocturnal asthma.
Risk factors for GORD?
1) Lifestyle (smoking, alcohol, obesity, stress)
2) Lower esophageal stricture
3) Hiatus hernia - stomach goes diaphragm hole.
Investigations for GORD?
1) 24h oesophageal pH monitoring
2) H-pylori screen
3) Endoscopy - dysphagia OR suspected peptic ulcer (>55 + ALARMS symptoms)
Managing GORD?
1) Judge if specialist referral (e.g. Chest Pain = CV)
2) Lifestyle (smoking/alcohol cessation, weight loss)
3) Medication
Complications of GORD
1) Oesophagitis - ulceration/stricture
2) Barrett’s Oesphagus > Oesphageal Cancer
Medical Management of GORD?
1) Gaviscon (alginate)
2) PPI OD (long term if stricture forms)
3) H2 blocker (persistent disease)
